Roles & Responsibilities

• Provide compassionate, patient-centered care to long-term care residents with thorough assessments and ongoing monitoring.
• Administer medications and treatments safely, including gastrostomy tube feedings, urinary catheter care, nebulized treatments, wound vac therapy, CPAP, and tracheostomy care.
• Communicate promptly and clearly with physicians and nurse practitioners, reporting changes in resident status and collaborating on care plans.
• Support residents with activities of daily living (ADLs), always promoting dignity, comfort, and quality of life.
• Coordinate smooth admissions and discharges, ensuring accurate documentation in PointClick EMR.
• Adapt quickly to varied shifts and resident needs, bringing flexibility, empathy, and a team-oriented mindset to every day.

Qualifications

• Active Hawaii RN license (or ability to apply immediately upon acceptance and obtain license before start date)
• Minimum of 1 year of nursing experience, with long-term care or skilled nursing experience highly preferred
• Current BLS/BCLS certification required
• Proficiency with PointClick EMR is a plus
• Must be comfortable with varied shifts, including weekends and holidays
• Must be able to arrive the Thursday before the start date for the employee health appointment

Knowledge & Skills

• Strong clinical assessment and critical thinking skills
• Confident in medication administration, including scheduled and prn medications
• Skilled in wound care, including wound vac therapy, and managing ostomy and catheter devices
• Comfortable with respiratory support equipment such as CPAP, nebulizers, and tracheostomy care
• Excellent verbal and written communication for collaborating with medical teams and educating residents and families
• Proficient with electronic health records, specifically PointClick EMR
• Ability to prioritize and multitask in a fast-paced, dynamic long-term care environment
