Roles & Responsibilities

• Manage a patient caseload and develop individualized care plans with physicians, caregivers, and support networks.
• Conduct comprehensive assessments to identify case management needs and clinical triggers.
• Monitor care plan effectiveness, document interventions, and adjust goals to support patient progress.
• Lead discharge planning efforts to ensure smooth transitions and continuity of care.
• Integrate behavioral health and long-term services to enhance member outcomes.
• Maintain regular outreach and ongoing management for assigned patients.

Qualifications

• Active Montana RN license
• American Heart Association BLS certification
• Minimum 2 years of nursing experience
• Utilization review experience required
• Bachelor's degree in nursing preferred
• 2 years of acute care case management experience preferred

Education

• Bachelor's Degree in Nursing (preferred)

Knowledge & Skills

• Sound judgment and the ability to stay calm in a busy environment
• Strong interpersonal, verbal, and written communication skills
• Tact, discretion, and confidentiality
• Proficiency with MS Office, EMR, and standard office equipment
• Strong organization and time management skills
• Emotional intelligence and teamwork skills
