Roles & Responsibilities

• Conduct comprehensive patient assessments and admit patients to hospice services
• Develop individualized plans of care addressing physical, psychosocial, environmental, safety, and developmental needs
• Coordinate care with a multidisciplinary team to ensure holistic support for patients and families
• Document observations and interventions accurately to maintain continuity of care
• Participate in quality assessment and performance improvement initiatives
• Conduct follow-up visits as needed to ensure patient comfort and care plan effectiveness

Qualifications

• Active California RN license
• BLS/BCLS certification required
• Minimum of one year of hospice experience
• Must hold a currently active state license
• Compassionate, patient-centered approach to care
