RN Case Manager - Hospice
Location: Sunnyvale, CA
Start Date: 09-08-2026
Contract Type: Travel Contract
Shift Type: Days, 8:30 AM - 5:00 PM (flexible based on patient availability; one weekend per month)
Hours per Week: 40
Candidate Pay Rate: $60 per hour

Roles & Responsibilities
- Conduct comprehensive patient assessments and admit patients to hospice services
- Develop individualized plans of care addressing physical, psychosocial, environmental, safety, and developmental needs
- Coordinate care with a multidisciplinary team to ensure holistic support for patients and families
- Document observations and interventions accurately to maintain continuity of care
- Participate in quality assessment and performance improvement initiatives
- Conduct follow-up visits as needed to ensure patient comfort and care plan effectiveness
Qualifications
- Active California RN license
- BLS/BCLS certification required
- Minimum of one year of hospice experience
- Must hold a currently active state license
- Compassionate, patient-centered approach to care
Knowledge & Skills
- Patient assessment and care planning
- Hospice and end-of-life care principles
- Care coordination and interdisciplinary collaboration
- Accurate clinical documentation
- Effective communication with patients, families, and team members
