Shift Type: Monday-Friday, 8:00 AM - 4:30 PM

Hours per Week: 40

Candidate Pay Rate: $53 per hour

Roles & Responsibilities

Coordinate seamless patient discharges to ensure smooth transitions home
Master case management using Meditech and Midas systems
Collaborate with RN and SW teams to optimize patient care plans
Tackle complex discharge planning with confidence and expertise
Advocate for patients managing ratios up to 25:1 with poise

Qualifications

Active RN license in good standing
At least 1 year of case management experience
Proficiency in Midas and Meditech required - must-haves for success here
Florida RN license or ability to obtain quickly for travel pros

Education

RN degree from accredited nursing program

Knowledge & Skills

Expertise in case management fundamentals
Hands-on experience with Midas and Meditech EMR systems
Strong skills in complex discharge planning
Team player mentality working alongside RN CM and SW CM colleagues
