Roles & Responsibilities

• Perform comprehensive patient assessments to understand each patient's unique health needs
• Develop and update individualized plans of care in partnership with patients and families
• Provide skilled nursing care and interventions that make a real difference
• Monitor patient progress and communicate changes to the care team promptly
• Educate patients and families on medications, treatments, and care plans
• Coordinate care with LPNs, therapists, caregivers, and providers for seamless support
• Document accurately and timely in the EMR system
• Participate in the on-call rotation to ensure patients have access to care when needed

Qualifications

• Active Registered Nurse (RN) license in Colorado or active NLC compact license
• BLS/BCLS certification required
• Valid driver's license, reliable transportation, and auto insurance
• Strong assessment and critical thinking skills
• Ability to work independently and manage a structured caseload
• Clear, professional communication with patients and team members
• Comfort with documentation and EMR systems (HCHB preferred)
• Home health experience preferred, but not required - new grads welcome!
