Roles & Responsibilities

• Provide compassionate hospice nursing care across settings including private homes, nursing homes, assisted living and group homes
• Assess patients thoroughly and manage complex symptoms with skill and empathy
• Serve as the primary RN for an assigned caseload and lead the interdisciplinary team caring for those patients
• Educate patients and caregivers to build confidence in managing care at home
• Develop, implement, and update individualized care plans based on patient goals and needs
• Coordinate care with physicians, social workers, chaplains, and other team members to ensure holistic support
• Document clinical findings and interventions accurately and timely
• Participate in occasional weekend backup and on-call coverage to ensure continuity of care

Qualifications

• Current Registered Nurse (RN) license in Colorado or eligibility to obtain one
• Comfort and experience providing care in patient homes and long-term care settings
• Strong assessment, clinical judgment, and symptom management skills
• Excellent communication and teaching skills for working with patients and family caregivers
• Ability to lead and collaborate within an interdisciplinary team
• Willingness to provide occasional weekend backup and on-call coverage

Education

• Associate Degree in Nursing (ADN) or Bachelor of Science in Nursing (BSN)

• Thorough clinical assessment and critical thinking
• Patient and family education techniques
• Care coordination and interdisciplinary collaboration
• Compassionate bedside manner and resilience in emotionally charged situations
