Contract Type: Contract

Shift Type: Monday-Friday, 8:00 AM - 5:00 PM with on-call rotation for evenings and weekends

Hours per Week: 40

Candidate Pay Rate: $62 per hour

Roles & Responsibilities

Conduct initial assessments of clients and families to determine home health needs
Develop and implement care plans with therapeutic, preventive, and rehabilitative nursing actions
Administer medications and treatments as prescribed by physicians
Provide health care instructions to clients and families
Coordinate client care and communicate with physicians and other healthcare professionals
Evaluate and supervise home health aide care

Qualifications

Current RN licensure
BLS certification
Experience in Home Health and Hospice care
Valid Driver's License and auto insurance

Education

Associate's or Bachelor's degree in Nursing

Knowledge & Skills

Home Health and Hospice care principles
EMR systems (HomeCare HomeBase)
Case management and care planning
Communication and interpersonal skills
Medication administration and treatment management
