Shift Type: Monday-Friday, 8:00 AM - 5:00 PM

Hours per Week: 40

Candidate Pay Rate: $70 per hour

Roles & Responsibilities

Provide compassionate physical therapy services to patients in their homes across Tehama County
Conduct Start of Care visits for orthopedic and geriatric patients when nursing is not ordered
Develop personalized treatment plans for patients recovering from joint replacements and managing chronic conditions like CHF and COPD
Complete approximately 5 meaningful patient visits each day while building lasting connections in the community
Document care efficiently using the HomeCare HomeBase EMR system on your provided tablet

Qualifications

Current Physical Therapy license in California
BLS certification required
Home Health experience preferred but will consider candidates with less than 2 years of experience who have Home Health background
Strong communication skills and ability to work independently

Knowledge & Skills

Physical Therapy assessment and treatment planning
Home Health documentation and compliance
Geriatric and orthopedic patient care
Managing patients with multiple chronic conditions
Independent decision-making in home settings
