Contract Type: Travel Contract

Shift Type: Monday-Friday, 8:00 AM - 5:00 PM with weekend rotation

Hours per Week: 40

Candidate Pay Rate: $46 per hour

Roles & Responsibilities

Provide skilled nursing care to adult patients in their homes across Marion, Washington, Taylor, LaRue, Hardin, Meade, and Breckenridge counties
Perform start of care, recertifications, and daily visits - manage a caseload of about 30 patients
Use Homecare Homebase (HCHB) for charting and documentation
Collaborate with clinical managers and educators for support and guidance
Ensure compliance with all state and federal regulations

Qualifications

Active RN license in the state of Kentucky (or compact license)
Minimum 1 year of Med-Surg experience in a hospital setting
At least 1 year of Home Health experience required
Proficiency with Homecare Homebase (HCHB) charting system
Current BLS certification

Knowledge & Skills

Strong clinical assessment and critical thinking skills
Excellent communication and patient education abilities
Comfortable working independently in a home health setting
Time management and organizational skills to handle 6.5 points per day
