Roles & Responsibilities

• Evaluate and treat patients to improve their daily living skills
• Develop personalized therapy plans that make a real difference
• Collaborate with a multidisciplinary team for holistic care
• Document patient progress and adjust treatments as needed
• Educate patients and families on home exercise programs

Qualifications

• Active Occupational Therapist license in New Jersey (or compact eligible)
• At least 1 year of experience in a skilled nursing or rehab setting
• Strong communication and patient-centered care skills
• Willingness to travel and adapt to new environments

Knowledge & Skills

• Expertise in functional assessments and treatment planning
• Knowledge of geriatric rehabilitation and fall prevention
• Proficiency with electronic medical records (EMR) systems
• Ability to work independently and as part of a care team
