Roles & Responsibilities

• Provide skilled physical therapy interventions under the direction of the supervising therapist to improve patient mobility and function
• Deliver compassionate, patient-centered care in a skilled nursing facility setting
• Assess patient progress and modify treatment plans to meet goals and safe discharge outcomes
• Document treatment sessions thoroughly and accurately in the patient record
• Collaborate with nursing, therapy colleagues, and interdisciplinary team members to coordinate care
• Educate patients and families on home programs, safety, and fall prevention to maximize independence

Qualifications

• Active or eligible PTA license for practice in Maryland
• Graduate of an accredited PTA program (or eligible to sit for licensure)
• Basic Life Support (BLS/CPR) certification preferred or able to obtain
• Comfortable working in a skilled nursing facility environment
• Excellent communication skills and ability to collaborate with a care team
• New grads welcome — any level of experience considered

Knowledge & Skills

• Foundational knowledge of therapeutic exercise and modalities
• Skilled in mobility training, transfers, and fall prevention strategies
• Proficient documentation and clinical reasoning skills
• Ability to adapt treatment plans for complex geriatric conditions
• Strong teamwork and patient advocacy abilities
