Physical Therapist - Skilled Nursing Facility

Candidate Pay Rate: $85 - $59.5 per hour

Roles & Responsibilities
- Evaluate patients and develop individualized treatment plans that drive recovery
- Provide skilled therapy focused on mobility, strength, balance, and functional independence
- Document patient progress and treatment outcomes accurately and in a timely manner
- Collaborate with nurses, CNAs, therapists, and other clinicians to ensure coordinated care
Qualifications
- Active California Physical Therapist license (required)
- Experience in Skilled Nursing Facility (SNF) or similar settings preferred
- Strong clinical reasoning and patient-centered treatment planning skills
- Excellent documentation and communication skills
- Ability to work independently and contribute as a collaborative team member
Knowledge & Skills
- Proven expertise in physical therapy evaluation and treatment planning
- Skilled in mobility, strength training, balance interventions, and functional training
- Clear, timely clinical documentation and outcome tracking
- Strong interpersonal skills for interdisciplinary collaboration
- Comfort working in a fast-paced SNF rehab environment
