Roles & Responsibilities
- Evaluate patients and develop individualized treatment plans that make a real difference
- Provide hands-on therapy and functional mobility training to restore independence
- Monitor and document patient progress with clear, timely notes
- Collaborate closely with the interdisciplinary healthcare team to coordinate care
- Ensure compliance with facility and regulatory standards to maintain a safe environment
Qualifications
- Active California Physical Therapist (PT) license (required)
- Current BLS (Basic Life Support) certification
- SNF (Skilled Nursing Facility) experience preferred
- Strong clinical judgment and excellent communication skills
Knowledge & Skills
- Physical Therapy assessment and intervention
- Patient evaluation and individualized treatment planning
- Hands-on therapeutic techniques and functional mobility training
- Progress monitoring and clinical documentation
- Interdisciplinary collaboration and care coordination
- Facility and regulatory compliance
- Patient-centered care and effective communication with patients and families
