Roles & Responsibilities

• Conduct thorough initial evaluations of patients in the hospital-at-home setting
• Develop and implement personalized treatment plans focused on functional goals
• Provide therapeutic exercises and manual therapy to promote recovery and independence
• Monitor patient progress and adjust care plans as needed
• Educate patients and families on safe movement and home exercise programs
• Collaborate with the care team and document all services in EPIC

Qualifications

• Current Michigan Physical Therapist (PT) license or eligibility to obtain one
• BLS certification (must be active)
• Experience with EPIC electronic health record is required
• Previous hospital-at-home, home health, or acute care experience preferred
• Ability to pass a background check, drug screen, and Trinity Health pre-employment requirements
• Up-to-date immunizations and health screenings as required by the facility

Knowledge & Skills

• Strong knowledge of physical therapy assessment and treatment techniques
• Proficiency in EPIC charting and documentation
• Excellent communication and patient education skills
• Ability to work independently and manage a daily schedule
• Compassionate, patient-centered approach to care
