Contract Type: Contract

Shift Type: Monday-Friday, 8:00 AM - 5:00 PM with every 3rd weekend rotation

Hours per Week: 40

Candidate Pay Rate: $64 per hour

Roles & Responsibilities

Deliver personalized physical therapy services in patients' homes across your assigned region
Assess and treat a variety of conditions including orthopedic injuries, strokes, joint replacements, and spinal conditions
Complete OASIS documentation to ensure compliance with government requirements
Collaborate with interdisciplinary teams including OT and Speech Therapy to optimize patient outcomes
Educate patients and families on home exercise programs and recovery strategies

Qualifications

Active Physical Therapist license in California
BLS certification required
At least one year of clinical experience as a Physical Therapist
Experience with OASIS documentation required
Home health experience preferred but not required

Knowledge & Skills

Strong assessment and clinical reasoning skills
Proficiency with Cerner EMR system
Excellent communication and patient education abilities
Ability to work independently in home-based settings
Compassionate approach to patient-centered care
