Physical Therapist Assistant - Home Health
Location: Grand Junction, CO

Contract Type: Travel Contract
Shift Type: Flexible Schedule - Full-time or part-time, hours vary
Candidate Pay Rate: $35 per hour

Roles & Responsibilities
- Provide skilled therapy services in patients' homes, making a real difference in their daily lives
- Carry out established treatment plans including therapeutic exercise, gait training, and balance activities
- Monitor and communicate patient progress to the supervising physical therapist
- Encourage and motivate patients to actively participate in their goals
- Document care accurately and timely in the EMR system
- Collaborate with a team of therapists, nurses, and caregivers to ensure holistic care
Qualifications
- Current Colorado Physical Therapist Assistant (PTA) license in good standing
- Graduate of an accredited Physical Therapist Assistant program
- Valid driver's license, reliable transportation, and auto insurance
- Strong communication and documentation skills
- Ability to work independently and adapt professionally in a home-based setting
- Certifications: ACLS, BLS/BCLS, PALS, NRP (required)
- New graduates and experienced PTAs welcome - less than 1 year of experience required
Education
- Graduate of an accredited Physical Therapist Assistant program

- Patient care and treatment plan implementation
- Therapeutic exercise and gait training
- Balance activities and functional mobility
- EMR documentation
- Patient communication and education
- Ability to work independently and as part of a collaborative team
