Roles & Responsibilities

• Provide skilled occupational therapy evaluations and evidence-based interventions for adult and/or pediatric patients as assigned
• Develop individualized treatment plans that promote functional independence and meaningful activity
• Collaborate closely with nursing, PT, SLP and other interdisciplinary team members to coordinate care
• Document patient status, progress and discharge planning thoroughly and on time
• Adapt treatment approaches and equipment recommendations based on patient goals and safety needs
• Float between assigned facilities (Herrin, MHC and St. Joe's) while maintaining continuity and quality of care
• Communicate clearly and compassionately with patients and families to set expectations and celebrate progress

Qualifications

• Current Illinois Occupational Therapist license (required)
• OTR/L credential preferred or required per facility
• Basic Life Support (BLS/CPR) certification required or obtain upon hire
• Comfortable floating between nearby facilities and working a flexible schedule (weekdays + weekends)
• Strong clinical assessment, documentation and time-management skills
• Previous acute care, inpatient rehab or similar clinical experience is preferred

Education

• Degree in Occupational Therapy (BS/BA in OT, Master's in OT, or OTD) as required for licensure
• OTR/L or equivalent credential required

Knowledge & Skills

• Proficiency with occupational therapy assessments and standardized outcome measures
• Skilled in ADL/IADL training, adaptive equipment recommendations and home safety assessments
• Familiarity with documentation systems and timely charting practices
• Ability to collaborate within an interdisciplinary team and communicate clearly with patients and families
• Flexibility to adjust treatment plans across different site workflows and patient populations
