Roles & Responsibilities

• Provide compassionate direct care to residents, helping with activities of daily living and promoting dignity.
• Assist with safe transfers, mobility, and positioning to maintain comfort and prevent injury.
• Take and record vital signs and report changes in condition to the care team promptly.
• Follow Trinity skills checklist and specialty competency testing standards to ensure high-quality care.
• Complete assigned documentation accurately and timely to support continuity of care.
• Adhere to infection prevention protocols and facility policies, including immunization and screening requirements.
• Collaborate with nursing staff and interdisciplinary team members to deliver person-centered care.

Qualifications

• Active Certified Nursing Assistant (CNA) credential (state-certified) required.
• Basic Life Support (BLS) certification required.
• Ability to pass Trinity specialty-specific competency testing and Trinity skills checklist.
• Proof of required immunizations and health screening: MMR, Hepatitis B, Varicella, Tdap, COVID-19, Influenza.
• 2-step PPD or IGRA (Quantiferon) TB testing and Trinity color vision clearance as required.
• Successful completion of Trinity 10-panel drug screen and license/certificate verification.

Education

• Certified Nursing Assistant (CNA) certification (state-required).

Knowledge & Skills

• Strong patient care fundamentals and hands-on experience assisting with ADLs.
• Good communication skills for working with residents, families, and clinical teams.
• Basic clinical assessment skills: vitals, observation, and reporting.
• Familiarity with infection control and safe handling/transfer techniques.
• Comfort with documentation and following facility-specific checklists and protocols.
• Dependability, attention to detail, and the ability to work independently on per-diem shifts.
