Roles & Responsibilities

• Provide compassionate, skilled nursing care to patients in their homes, delivering clinical excellence every visit
• Perform thorough patient assessments and monitor vital signs to guide care decisions
• Administer medications and treatments as ordered, ensuring patient safety and adherence
• Assist with wound care and chronic disease management to support healing and quality of life
• Educate patients and families on care plans and medications so they feel confident at home
• Maintain accurate and timely documentation in Cerner to ensure continuity of care
• Collaborate with the interdisciplinary healthcare team to deliver coordinated, patient-centered care

Qualifications

• Active California LVN license (required for submission)
• Minimum 2 years of nursing experience
• BLS certification (required)
• Home Health experience preferred
• Strong documentation and communication skills, Cerner experience preferred
• Able to manage time effectively and work independently in patient homes

Education

• Completion of an accredited vocational nursing program or equivalent (LVN)

Knowledge & Skills

• Patient assessment and vital sign monitoring
• Medication administration and treatment implementation
• Wound care and chronic disease management
• Proficient documentation practices, familiarity with Cerner preferred
• Patient and family education skills
