Roles & Responsibilities

• Provide compassionate, patient-centered nursing care consistent with LPN scope of practice
• Administer medications and treatments accurately and document appropriately
• Monitor and report changes in patient condition to the RN or provider promptly
• Collaborate with the interdisciplinary team to support individualized care plans
• Maintain clear, timely, and accurate documentation in the medical record
• Assist residents with activities of daily living while promoting dignity and independence
• Adhere to infection control practices and facility safety protocols
• Participate in meetings and trainings to stay current with best practices

Qualifications

• Current LPN license (Licensed Practical Nurse) — Pennsylvania licensure expected
• Completion of an accredited practical nursing program
• Current basic life support (BLS/CPR) certification preferred or willing to obtain
• Comfortable working in a long-term care or skilled nursing environment; prior experience is a plus
• Strong communication skills and ability to work well within a multidisciplinary team
• Reliable, professional, and patient-focused attitude

Education

• Diploma or certificate from an accredited practical nursing program

Knowledge & Skills

• Medication administration and basic clinical procedures
• Patient assessment and vital signs monitoring
• Accurate clinical documentation and charting
• Infection control and standard precautions
• Time management and ability to prioritize care tasks
• Effective communication with patients, families, and care teams
