Roles & Responsibilities

• Provide compassionate direct patient care, ensuring dignity and comfort for each resident
• Administer medications and treatments accurately and document all care in the medical record
• Monitor patient condition, record vital signs, and promptly report changes to the nurse leader
• Collaborate with RNs, CNAs, therapists, and interdisciplinary teams to deliver coordinated care
• Support admissions, discharges, and transfers with clear communication and excellent bedside manner
• Assist with wound care, IV care as applicable to LPN scope, and help maintain infection control standards
• Mentor and support nursing assistants, promoting a positive, safe work environment

Qualifications

• Active LPN license in Pennsylvania (required)
• Graduation from an accredited Practical Nursing program
• Current BLS/CPR certification (or willingness to obtain upon hire)
• Previous experience in skilled nursing or long-term care preferred
• Strong communication skills and a patient-centered approach
• Ability to pass background checks and required health screenings

Education

• Graduated from an accredited Practical Nursing (LPN) program and active LPN license

Knowledge & Skills

• Medication administration and accurate charting
• Basic wound care and infection control practices
• Ability to take and interpret vital signs and recognize changes
• Familiarity with electronic health records and documentation best practices
• Team collaboration, time management, and prioritization in a fast-paced setting
• Compassionate patient and family communication
