Roles & Responsibilities

• Provide compassionate, safe nursing care to patients in the assigned unit
• Administer medications and treatments accurately and document per policy
• Perform focused patient assessments and communicate changes to the care team
• Assist with care planning and patient education to promote recovery and independence
• Collaborate with nurses and other clinicians to ensure seamless, high-quality care
• Maintain accurate, timely documentation in the patient record and follow facility protocols
• Support safe transitions of care, including handoffs and discharge teaching

Qualifications

• Active LPN license (practical/vocational nurse) in the state of practice
• Minimum current BLS/CPR certification or willingness to obtain prior to start
• Clinical experience in a long-term care or similar setting preferred
• Current immunizations and ability to pass required background and health screenings
• Strong interpersonal skills and the ability to work effectively on a night-shift team

Education

• Completion of an accredited Practical Nursing program (diploma or certificate) and active LPN license

Knowledge & Skills

• Medication administration and basic nursing procedures
• Patient assessment and vital signs monitoring
• Wound care and common clinical treatments appropriate for LPN scope
• Clear, timely clinical documentation and charting
• Infection prevention and safe patient handling practices
• Effective communication and teamwork with multidisciplinary staff
