Roles & Responsibilities

• Provide compassionate, day-to-day nursing care to residents in a skilled nursing setting
• Administer medications, treatments, and injections as ordered by providers
• Monitor and document vital signs, symptoms, and changes in condition
• Collaborate with RNs, physicians, and the care team to update care plans
• Educate residents and families on treatment plans and health management
• Maintain accurate and timely patient records in the EMR

Qualifications

• Active Licensed Practical Nurse (LPN) license in New Hampshire
• Current BLS/CPR certification
• COVID-19 vaccination or approved medical/religious exemption
• Strong clinical assessment and communication skills
• Previous long-term care or skilled nursing experience is a plus but not required

Knowledge & Skills

• Medication administration and documentation
• Wound care and infection control practices
• Resident assessment and care planning
• Electronic medical records (EMR) familiarity
• Teamwork, empathy, and clear communication
