Roles & Responsibilities

• Provide compassionate physical therapy services to patients in their homes
• Perform comprehensive OASIS assessments and Start of Care visits
• Develop and implement individualized treatment plans to restore mobility and independence
• Collaborate with the interdisciplinary care team for optimal patient outcomes
• Accurately document patient progress in Homecare Homebase (HCHB)
• Educate patients and families on therapeutic exercises and fall prevention

Qualifications

• Active Physical Therapist (PT) license in Indiana - must be in hand upon submission
• Minimum of 1 year of home health experience
• Proficiency with Homecare Homebase (HCHB) charting system
• Current BLS certification (AHA)
• Experience with OASIS assessments and Start of Care
• Reliable transportation and valid driver's license for home visits

Knowledge & Skills

• Strong patient assessment and clinical reasoning
• Excellent communication and interpersonal skills
• Knowledge of home health regulations and Medicare conditions of participation
• Ability to work independently and manage a daily schedule
• Compassionate, patient-centered approach to care
