Roles & Responsibilities

• Provide compassionate physical therapy care to patients in the comfort of their own homes
• Develop and implement individualized treatment plans to help patients regain mobility and independence
• Collaborate with a multidisciplinary team to ensure holistic patient care
• Educate patients and their families on therapeutic exercises and techniques
• Maintain accurate patient records using Cerner charting system

Qualifications

• Active California Physical Therapist license
• BLS certification from AHA or ARC
• Minimum 2 years of experience as a Physical Therapist
• Experience in home health setting is a plus
• Willingness to travel up to 25% for patient visits

Knowledge & Skills

• Strong clinical knowledge of physical therapy practices and methodologies
• Proficiency in home health care coordination
• Experience with Cerner charting system
• Excellent communication and interpersonal skills
• Ability to work independently and manage a caseload
