Roles & Responsibilities

• Provide comprehensive physical therapy assessments and treatments in patients' homes, assisted living facilities, senior apartments, and board and care homes.
• Perform Start of Care visits and evaluations, managing 3-5 patients per day.
• Develop and implement individualized care plans in collaboration with the healthcare team.
• Supervise and guide up to two Physical Therapist Assistants (PTAs) and COTAs to ensure quality care.
• Document patient progress and care coordination accurately using HomeCare Home Base (HCHB).
• Adapt to varied and sometimes challenging home environments while maintaining professional standards.

Qualifications

• Must hold an active California Physical Therapist license.
• Current BLS certification (AHA) and NPI number.
• Minimum 2 years of experience in a healthcare setting; community/home health experience preferred.
• Home Health experience including OASIS required.
• Proficiency with HomeCare Home Base (HCHB) preferred.
• Valid driver's license and proof of automobile insurance.

Knowledge & Skills

• Strong clinical knowledge in physical therapy across ortho, cardio, pulmonary, oncology, neuro, geriatric, and amputee populations.
• Excellent oral and written communication skills for coordinating with patients, families, and interdisciplinary teams.
• Organizational skills to manage a caseload of 3-5 patients daily and travel efficiently.
• Experience with OASIS documentation and home health regulations.
• Ability to work in diverse home environments, sometimes cramped, hot/cold, or cluttered.
