Roles & Responsibilities

• Provide skilled, compassionate nursing care to residents in accordance with care plans and facility policies
• Administer medications and treatments safely and accurately, documenting all interventions
• Perform assessments and report changes in patient condition promptly to the care team
• Collaborate with RNs, physicians, therapists, and support staff to coordinate patient-centered care
• Educate patients and families on procedures, medications, and care plans with clarity and empathy
• Maintain accurate, timely documentation in the electronic health record and assist with transitions of care

Qualifications

• Current LPN license in good standing (New Hampshire licensure preferred or compact/reciprocal where applicable)
• BLS (Basic Life Support) certification required; ACLS or other certifications encouraged if available
• Current immunizations per facility policy and ability to pass background and health screenings
• Comfortable working 12+ hour day shifts and able to lift/transfers as required by patient needs
• Strong communication skills, attention to detail, and a team-oriented mindset

Education

• LPN diploma or certificate from an accredited nursing program; state licensure required

Knowledge & Skills

• Proficient in medication administration, wound care basics, and routine nursing assessments
• Familiarity with electronic health records and accurate, timely documentation
• Ability to prioritize tasks in a fast-paced long-term care environment
• Skilled in patient/family education, compassionate bedside manner, and teamwork
• Basic knowledge of infection control, safety protocols, and regulatory compliance
