Roles & Responsibilities

• Provide compassionate, hands-on nursing care to patients as a Licensed Practical Nurse
• Administer medications and treatments accurately and safely according to orders
• Perform routine assessments and monitor patient status, reporting changes promptly
• Document care clearly and timely in the patient record to support continuity
• Collaborate with RNs, providers, and the broader care team to optimize outcomes
• Support patient and family education with empathy and clear communication
• Follow infection control and facility policies to maintain a safe environment

Qualifications

• Active Licensed Practical Nurse (LPN) license required
• Graduation from an accredited practical nursing program
• Current BLS/CPR certification required or able to obtain promptly
• Comfortable with medication administration, basic assessments, and documentation
• Reliable, professional, and able to meet facility onboarding requirements (background check, immunizations, etc.)
• Previous LPN experience in acute or long-term care is a plus but motivated new grads welcome

Education

• Diploma or certificate from an accredited Practical Nursing program (LPN)

Knowledge & Skills

• Strong basic nursing skills: vital signs, injections, wound care basics
• Accurate medication administration and patient monitoring
• Clear, concise clinical documentation and handoff communication
• Ability to prioritize tasks in a dynamic clinical environment
• Team collaboration and a patient-centered approach to care
• Comfort using electronic medical records and facility documentation systems
