Roles & Responsibilities

Provide compassionate direct patient care, ensuring comfort and dignity at every step
Administer medications and treatments accurately and document each intervention
Monitor and record vital signs, recognizing and escalating changes in patient condition
Collaborate with nurses and interdisciplinary team members to coordinate individualized care
Maintain clear, timely documentation in the patient chart and communicate updates to the team
Support admission, discharge, and bedside procedures while educating patients and families

Qualifications

Active LPN license in Pennsylvania (required)
Completion of an accredited practical nursing program
Current Basic Life Support (BLS/CPR) certification or willingness to obtain
Strong clinical assessment skills and attention to detail
Excellent communication and teamwork skills; patient-centered attitude
Ability to pass background check and any required screenings

Education

Completion of an accredited Practical Nursing (LPN) program
High school diploma or GED

Knowledge & Skills

Solid knowledge of medication administration and infection control practices
Skilled in taking and interpreting vital signs and basic clinical assessments
Comfortable with electronic medical records and accurate documentation
Ability to prioritize tasks, manage time under pressure, and advocate for patients
Familiarity with long-term care and resident-centered approaches preferred
