Roles & Responsibilities
- Provide compassionate direct patient care, ensuring comfort and dignity at every step
- Administer medications and treatments accurately and document each intervention
- Monitor and record vital signs, recognizing and escalating changes in patient condition
- Collaborate with nurses and interdisciplinary team members to coordinate individualized care
- Maintain clear, timely documentation in the patient chart and communicate updates to the team
- Support admission, discharge, and bedside procedures while educating patients and families
Qualifications
- Active LPN license in Pennsylvania (required)
- Completion of an accredited practical nursing program
- Current Basic Life Support (BLS/CPR) certification or willingness to obtain
- Strong clinical assessment skills and attention to detail
- Excellent communication and teamwork skills; patient-centered attitude
- Ability to pass background check and any required screenings
Education
- Completion of an accredited Practical Nursing (LPN) program
- High school diploma or GED
Knowledge & Skills
- Solid knowledge of medication administration and infection control practices
- Skilled in taking and interpreting vital signs and basic clinical assessments
- Comfortable with electronic medical records and accurate documentation
- Ability to prioritize tasks, manage time under pressure, and advocate for patients
- Familiarity with long-term care and resident-centered approaches preferred
