Candidate Pay Rate: $56 per hour

Roles & Responsibilities

• Evaluate patients in their homes and develop personalized treatment plans
• Help patients regain mobility, strength, and independence through therapeutic exercises
• Educate patients and families on safe movement and home exercise programs
• Document patient progress accurately and communicate with the care team
• Adapt treatment approaches to meet each patient's unique needs and environment
• Coordinate with physicians, nurses, and other therapists to ensure comprehensive care

Qualifications

• Current BLS certification required
• Active and unencumbered Physical Therapist license in Iowa or compact eligibility
• Previous home health or outpatient physical therapy experience preferred
• Must be comfortable traveling to patient homes within the Monticello area
• Epic EHR experience preferred but not required

Knowledge & Skills

• Strong clinical knowledge of physical therapy assessment and treatment techniques
• Excellent communication and patient education skills
• Ability to work independently and manage a flexible daily schedule
• Compassionate and patient-centered approach
• Proficiency with electronic health records, especially Epic
