Roles & Responsibilities

• Provide compassionate, skilled nursing care to residents in a Skilled Nursing Facility
• Administer medications and treatments safely and accurately according to policy
• Monitor and document resident condition, reporting changes to the care team
• Assist residents with activities of daily living and promote independence
• Participate in care planning and collaborate with nurses, therapists, and families
• Maintain accurate and timely electronic and paper health records
• Follow infection prevention protocols, including respirator and fit testing requirements
• Support a positive, patient-centered environment and advocate for resident needs

Qualifications

• Active LPN license (must be valid in Michigan for Michigan assignments)
• BLS (Basic Life Support) certification
• Experience in Skilled Nursing Facility (SNF) nursing or long-term care preferred
• Ability to join the per diem pool and work variable shifts as needed
• Successful completion of required health screenings and immunizations (PPD/IGRA, MMR, Hep B, Varicella, Tdap, COVID-19, Influenza)
• Ability to pass Trinity 10 Panel Drug Screen, respirator evaluation, and specialty competency testing

Education

• Graduated from an accredited LPN/LVN program (practical nursing diploma or certificate)

Knowledge & Skills

• Strong clinical assessment skills and sound judgment in acute and chronic care
• Familiarity with medication administration, wound care basics, and ADL support
• Comfortable with electronic medical records and timely documentation
• Team-player attitude with excellent communication skills
• Knowledge of infection control practices and respirator use
• Ability to adapt quickly to different units and resident populations
