Roles & Responsibilities

• Collaborate closely with RN Care Managers to support seamless patient care coordination
• Maintain accurate, timely documentation across CERNER, NATE, and ENSOCARE systems
• Communicate effectively with patients, families, and the care team to build trust and ensure clarity
• Shift attention between tasks and priorities to keep every patient's plan on track
• Be a steady, supportive presence for patients and families navigating their healthcare journey
• Help identify and address barriers to care, paving the way for positive outcomes

Qualifications

• At least 2 years of experience in a healthcare coordination or related role
• Strong documentation and communication skills
• Proven ability to work collaboratively with nurses, care managers, and multidisciplinary teams
• Ability to manage competing priorities and shift attention between tasks with ease
• A patient-centered approach that builds trust with patients, families, and teammates
• Experience with CERNER, NATE, or ENSOCARE is a plus

Knowledge & Skills

• Knowledge of electronic health records and care coordination workflows
• Strong interpersonal skills and the ability to build rapport
• Excellent organizational and time-management abilities
• Comfort with technology and documentation systems
• Understanding of patient care transitions and discharge planning
