Roles & Responsibilities

• Provide compassionate direct patient care including assistance with activities of daily living
• Monitor and report changes in patient condition to the nursing team promptly
• Assist with safe transfers, mobility support, and fall prevention measures
• Take and record vital signs accurately and communicate findings to clinical staff
• Maintain clean, safe patient environments and follow infection control protocols
• Document care and observations in the facility’s record system in a timely manner
• Work collaboratively with the interdisciplinary team to deliver person-centered care

Qualifications

• Current CNA certification (state-appropriate) required — certification/license must be active
• Basic Life Support (BLS/CPR) preferred or willing to obtain
• Ability to pass background check and meet facility immunization requirements
• Strong communication skills and a caring, patient-first attitude
• Availability to start on or around 2026-04-10 (start date provided in posting)

Education

• High school diploma or equivalent
• Completed state-approved CNA training program and current CNA certification

Knowledge & Skills

• Skilled in patient hygiene, feeding, toileting, and mobility assistance
• Comfortable taking and documenting vital signs and observing clinical changes
• Knowledge of basic infection control and safety procedures
• Ability to follow care plans and take direction from licensed nursing staff
• Team player with good time management and organizational skills
• Familiarity with electronic charting or willingness to learn facility systems
