Roles & Responsibilities

Provide compassionate, hands-on personal care to residents with dignity and respect
Assist with activities of daily living including bathing, dressing and feeding
Take and record vital signs and report changes to the nursing team promptly
Support safe transfers and mobility, using proper body mechanics and equipment
Maintain accurate and timely documentation of care and resident interactions
Collaborate with nurses and interdisciplinary team members to promote resident well-being
Follow infection prevention protocols and maintain a clean, safe environment
Build trusting relationships with residents and their families through clear communication

Qualifications

Current CNA certification (PA certification preferred or required)
CPR/BLS certification preferred or willingness to obtain
Ability to work 8-hour shifts and reliably report to Smethport facility
Comfortable with hands-on resident care and lifting with assistance
Successful completion of background check and any required health screenings
Strong communication skills and a team-first attitude

Education

High school diploma or GED preferred
Current CNA certification

Knowledge & Skills

Patient care basics and ADL assistance
Vital signs measurement and documentation
Basic infection control practices
Safe resident transfer and mobility techniques
Effective communication with residents, families and clinical staff
Time management and dependable attendance
Empathy, patience and strong observational skills
