Roles & Responsibilities

• Provide compassionate daily care to residents in a skilled nursing and long-term care setting
• Assist with activities of daily living (ADLs) such as bathing, dressing, grooming, and toileting
• Monitor and record vital signs and report changes to nursing staff
• Support residents with mobility, transfers, and positioning to ensure safety and comfort
• Document care provided accurately and communicate effectively with the care team
• Promote a warm, respectful, and safe environment for residents and families

Qualifications

• Active, unrestricted Colorado CNA certification (required)
• Current BLS/BCLS certification
• Minimum 4 years of long-term care (LTC) or skilled nursing experience
• Must be fully vaccinated per Colorado state mandate
• Willing to comply with facility housing policies (no pets, no smoking)
• Traveler perm address must be at least 65 miles from facility (no local travelers)

Knowledge & Skills

• Solid understanding of long-term care and skilled nursing practices
• Strong communication and interpersonal skills
• Ability to document accurately and follow care plans
• Compassionate, patient, and dependable
• Comfortable working in a small community facility
