Roles & Responsibilities
- Coordinate patient care across the hospital stay to ensure safe, timely discharges
- Perform utilization review and case management to align care with admission criteria and benefits eligibility
- Develop and execute discharge plans in collaboration with interdisciplinary teams (social work, providers, and support services)
- Manage appeals and denials processes and work payer issues proactively
- Conduct needs assessments, order DME when indicated, and handle prior authorizations
- Document thoroughly and accurately in Epic to support continuity of care and regulatory compliance
- Float as needed to support department staffing and maintain consistent patient coverage
Qualifications
- Minimum 2 years of acute hospital case management experience (must be current within the last year) — hard requirement
- Experience with care coordination, discharge planning, and utilization review
- Epic charting experience required
- Travel experience preferred; first-time travelers can be screened to manager
- Experience working with HMOs, IPAs, advance directives, POLST, and protective services processes
- Experience with appeals, denials, benefits eligibility, and DRG/CMS regulations
- Strong communication skills and comfort working with interdisciplinary teams
Education
- Preferred: BSN, BS in Social Work, or related degree; equivalent clinical case management experience accepted
Knowledge & Skills
- In-depth knowledge of admission criteria, benefits eligibility, and discharge planning
- Familiarity with CMS regulations, DRG methodology, HIPAA, and The Joint Commission standards
- Comfort managing worker's compensation cases and prior authorizations
- Ability to manage caseloads up to ~25 patients and prioritize effectively
- Skilled documentation in Epic and working within hospital case management workflows
