Roles & Responsibilities

• Coordinate patient care across the continuum — manage transitions from admission to safe discharge
• Perform utilization review and manage authorizations to ensure appropriate, timely care
• Develop and execute individualized discharge plans, including ordering DME and arranging post-acute services
• Assess benefits eligibility and collaborate with payers, HMOs and IPAs to resolve coverage issues
• Document thoroughly in Epic to maintain clear, auditable patient records
• Work with interdisciplinary teams (social work, therapy, physicians) to solve complex care needs
• Manage appeals and denials when necessary to advocate for patient access to services
• Float to support unit needs while maintaining high standards of case management

Qualifications

• Minimum 2 years of acute hospital case management experience (care coordination & discharge planning) — must be current within the last year
• Experience with travel assignments preferred; first-time travelers may be screened to manager
• Proficiency with Epic or similar electronic health record systems (charting required)
• Experience with HMOs/IPAs, advance directives/POLST, and protective services processes
• Demonstrated ability to manage prior authorizations, appeals, and denials
• Current clinical licensure as applicable (RN or other clinician license) or equivalent clinical background
• Strong communication and interdisciplinary collaboration skills

Education

• Clinical degree or relevant healthcare licensure preferred (e.g., RN or equivalent clinical qualification)

Knowledge & Skills

• Admission criteria and benefits eligibility assessment
• Needs assessment and ordering durable medical equipment (DME)
• Care coordination and discharge planning best practices
• Utilization review and DRG/CMS regulatory knowledge
• HIPAA and The Joint Commission/core measure awareness
• Worker's compensation case management familiarity
• Ability to work with high daily caseloads (up to ~25) and prioritize effectively
