Roles & Responsibilities
- Coordinate patient care across the continuum — manage transitions from admission to safe discharge
- Perform utilization review and manage authorizations to ensure appropriate, timely care
- Develop and execute individualized discharge plans, including ordering DME and arranging post-acute services
- Assess benefits eligibility and collaborate with payers, HMOs and IPAs to resolve coverage issues
- Document thoroughly in Epic to maintain clear, auditable patient records
- Work with interdisciplinary teams (social work, therapy, physicians) to solve complex care needs
- Manage appeals and denials when necessary to advocate for patient access to services
- Float to support unit needs while maintaining high standards of case management
Qualifications
- Minimum 2 years of acute hospital case management experience (care coordination & discharge planning) — must be current within the last year
- Experience with travel assignments preferred; first-time travelers may be screened to manager
- Proficiency with Epic or similar electronic health record systems (charting required)
- Experience with HMOs/IPAs, advance directives/POLST, and protective services processes
- Demonstrated ability to manage prior authorizations, appeals, and denials
- Current clinical licensure as applicable (RN or other clinician license) or equivalent clinical background
- Strong communication and interdisciplinary collaboration skills
Education
- Clinical degree or relevant healthcare licensure preferred (e.g., RN or equivalent clinical qualification)
Knowledge & Skills
- Admission criteria and benefits eligibility assessment
- Needs assessment and ordering durable medical equipment (DME)
- Care coordination and discharge planning best practices
- Utilization review and DRG/CMS regulatory knowledge
- HIPAA and The Joint Commission/core measure awareness
- Worker's compensation case management familiarity
- Ability to work with high daily caseloads (up to ~25) and prioritize effectively
