Candidate Pay Rate: $74 per hour

Roles & Responsibilities
- Provide compassionate care coordination for patients in need of mental health support
- Complete thorough 5150 evaluations to ensure patient safety during psychiatric holds
- Collaborate with interdisciplinary teams to develop individualized care plans
- Support patients and families through challenging mental health situations with empathy and expertise
Qualifications
- Active California LCSW license required
- Current BLS certification
- 5150 certification in California required
- Prior Dignity Health experience strongly preferred
Knowledge & Skills
- Expertise in involuntary psychiatric holds and mental health crisis intervention
- Strong assessment and documentation skills
- Ability to work collaboratively within a care coordination model
