Roles & Responsibilities
- Develop and manage patient care plans in collaboration with physicians and families
- Coordinate care through effective interdisciplinary communication
- Provide ongoing patient assessments and manage pain and symptoms
- Document patient progress and communicate with care teams
- Teach patients and caregivers essential care techniques
Qualifications
- Licensed Registered Nurse in Vermont and New Hampshire
- Graduate of an accredited school of nursing
- 2+ years of experience in medical/surgical/oncology nursing or hospice care
- Preferred experience in hospice care and case management
Education
- Accredited school of nursing graduate
