Location: White River Junction, VT
Contract Type: Perm

Candidate Pay Rate: $74 per hour

Roles & Responsibilities
- Assess patients in their homes to evaluate clinical, functional, and emotional status
- Develop, implement, and periodically review individualized care plans with patients, families, and physicians
- Coordinate care through interdisciplinary communication and timely case conferences
- Document evaluations, treatment goals, and progress in accurate clinical records
- Educate patients and caregivers on self-care techniques, medications, diet, and disease management
- Communicate significant changes in patient condition to physicians and the care team
- Advise other disciplines of planned discharges and ensure smooth care transitions
Qualifications
- Licensed Registered Nurse in Vermont or New Hampshire OR hold an unencumbered compact license
- Graduate of an accredited school of nursing
- Minimum 2 years of Home Health nursing experience
- Strong clinical assessment and documentation skills
- Excellent communication and interdisciplinary collaboration abilities
Education
- Graduate of an accredited school of nursing
Knowledge & Skills
- Home Health clinical practices and care planning
- Patient assessment and ongoing clinical evaluation
- Clinical documentation and electronic charting
- Interdisciplinary communication and case coordination
- Patient and caregiver education and health counseling
- Familiarity with hospice, rehabilitation, and discharge coordination (preferred)
