Candidate Pay Rate: $56 per hour

Roles & Responsibilities
- Provide compassionate physical therapy services to patients in their homes
- Conduct start of care assessments, recertifications, and resumption of care visits
- Deliver 5-6 meaningful patient visits per day, helping individuals regain independence
- Collaborate with a supportive clinical team available for guidance in the field
- Complete OASIS documentation and ensure quality care standards are met
- Perform occasional safety evaluations or teaching visits in hospice situations
Qualifications
- Current ND Physical Therapy License required
- BLS certification needed
- Valid Driver's License and Auto Insurance essential
- Home Health experience and OASIS training preferred
