Roles & Responsibilities
- Provide compassionate physical therapy services across four rural counties
- Conduct comprehensive Oasis Assessments for accurate patient evaluations
- Manage Start of Care cases with attention to detail and patient needs
- Deliver personalized treatment plans for elderly patients recovering from falls and fractures
- Document visits efficiently using modern tablet technology
- Build meaningful relationships with patients in their home environments
Qualifications
- Current Physical Therapist license in Indiana
- Experience with Oasis Assessments preferred
- Comfortable working independently in rural settings
- Strong communication and time management skills
- Valid driver's license and reliable transportation
