Roles & Responsibilities

• Deliver compassionate, patient-centered nursing care to residents using the nursing process
• Perform comprehensive assessments, plan, implement, and evaluate individualized care plans in partnership with residents and families
• Accurately document all nursing care, observations, and outcomes in a timely manner
• Collaborate with physicians, interdisciplinary team members, and support staff to ensure effective and efficient care delivery
• Provide resident and family education, respecting their unique age, cultural, and personal preferences
• Contribute to quality improvement initiatives that enhance resident outcomes and align with organizational mission and vision

Qualifications

• Active California Registered Nurse (RN) license
• Current CPR certification
• At least one (1) year of experience as a Registered Nurse
• Graduate of an accredited school of professional nursing
• A genuine passion for caring for seniors and long-term care residents

Education

• Solid understanding of the nursing process and evidence-based practice

• Proficient at electronic health records and clinical charting
• Excellent communication and interpersonal skills for working with residents, families, and care teams
• Critical-thinking and problem-solving abilities in a dynamic care environment
• Knowledge of Skilled Nursing Facility (SNF) regulations and quality standards
• Commitment to continuous learning and performance improvement
