Contract Type: Contract
Shift Type: Monday-Friday, 7:30 AM - 4:00 PM or 8:00 AM - 4:30 PM, up to every other weekend
Hours per Week: 40
Candidate Pay Rate: $71 per hour

Roles & Responsibilities
- Lead meaningful goals-of-care conversations that truly matter to patients and families
- Assess patient needs and develop personalized care plans with compassion
- Educate patients and families on disease processes, advance directives, and POLSTs
- Facilitate family meetings and provide emotional support during end-of-life discussions
- Collaborate with physicians and the interdisciplinary team during daily rounds
- Manage a rewarding caseload of 5-6 new patients and 3-4 follow-ups each day
Qualifications
- Active California RN license required
- 2-5 years of recent palliative or hospice experience
- BLS certification required
- ELNEC certification preferred
- Strong acute care, clinical thinking, and communication skills
- ICU or oncology experience is a plus
Knowledge & Skills
- Cerner EMR proficiency
- Palliative care screening and consultation expertise
- Advance care planning and family meeting facilitation
- Strong organizational and flexibility skills
