Contract Type: Contract
Shift Type: Rotating shifts with weekends required approximately every 4 weeks and all minor holidays plus 2 major holidays worked
Hours per Week: 40
Candidate Pay Rate: $71 per hour

Roles & Responsibilities
- Provide compassionate end-of-life care to hospice patients in their homes and facilities
- Manage a caseload of 12-14 patients with 4 visits or 2 start-of-care assessments daily
- Build trusting relationships with patients and families during their most vulnerable moments
- Collaborate with interdisciplinary teams to ensure comprehensive, dignified care
- Document care accurately using Home Care Home Base EMR system
Qualifications
- Active California RN license required
- Minimum 1 year of hospice experience
- Current BLS certification
- Strong communication and compassionate bedside manner
- Ability to work independently in home care settings
Knowledge & Skills
- Hospice and palliative care principles
- Home Care Home Base EMR documentation
- Patient and family education on end-of-life care
- Comfort care and symptom management
- Interdisciplinary team collaboration
