Contract Type: Contract

Shift Type: Monday-Friday, 8:00 AM - 5:00 PM with every other weekend rotation

Hours per Week: 40

Candidate Pay Rate: $82 per hour

Roles & Responsibilities

Coordinate comprehensive care plans for 15-20 patients daily across all hospital areas
Complete initial assessments and utilization reviews using Interqual criteria
Develop discharge plans that address the unique needs of all payers, uninsured, and homeless populations
Collaborate with hospitalists, intensivists, and interdisciplinary teams to optimize patient outcomes
Document efficiently in Cerner while managing competing priorities throughout each shift
Advocate for patients while balancing clinical needs with payer requirements

Qualifications

Active California RN license required
Current BLS certification
Recent experience with Dignity Health facilities strongly preferred
Strong collaboration, teamwork, and communication skills essential
Ability to reprioritize tasks effectively throughout the day
Two supervisory references required at time of submittal

Knowledge & Skills

Case Management and Care Coordination expertise
Utilization Review and Interqual criteria knowledge
Cerner electronic medical record proficiency
Discharge planning across diverse patient populations
Experience working with multiple payer types and uninsured patients
Strong organizational and time management abilities
