Roles & Responsibilities

• Conduct timely start-of-care visits so patients begin therapy when it matters most
• Perform comprehensive clinical assessments and design individualized, outcome-focused care plans
• Deliver clear patient and family education that promotes independence and meaningful recovery
• Supervise and mentor PTAs and clinical staff to elevate quality across your caseload
• Complete accurate start-of-care and follow-up documentation meeting next-day and 48-hour deadlines
• Coordinate care with intake teams, supervisors, and office staff to ensure smooth transitions
• Plan and manage your territory with efficient time and travel planning while meeting productivity goals
• Optimize visit mixes and convert productivity points to hours to maximize patient outcomes and efficiency

Qualifications

• Active California Physical Therapist (PT) license required
• Doctor of Physical Therapy (DPT) or equivalent degree leading to CA PT licensure
• Minimum 1 year of home health experience
• Valid driver’s license and reliable transportation for home visits (company car option available)
• Comfortable using mobile devices and documentation tools; Kinnser experience is a plus
• Strong documentation skills with the ability to meet next-day and 48-hour timelines
• Clear, professional communication skills with patients, families, and interdisciplinary teams
• Experience supervising PTAs or clinical staff is a plus

Education

• Doctor of Physical Therapy (DPT) or equivalent degree leading to California PT licensure

Knowledge & Skills

• Familiarity with home health workflows and OASIS-related visit types
• Skilled in clinical assessment, evaluation, and outcome-focused care planning
• Accurate, timely mobile documentation (Kinnser experience is a bonus)
• Time management and territory planning to optimize travel and visits
• Ability to convert productivity points to hours and manage visit mixes
• Strong patient and family education techniques that promote recovery
• Professional collaboration with interdisciplinary teams and office staff
• Comfortable mentoring and supervising PTAs and clinical staff
