Roles & Responsibilities

Conduct thorough patient evaluations to create personalized rehab plans
Deliver daily treatments that make a real difference in patients' lives
Manage discharges ensuring smooth transitions and lasting progress
Collaborate with a dynamic team to optimize patient outcomes in a skilled nursing setting

Qualifications

Active Physical Therapist license (PT or DPT)
1-3 years of experience, with prior SNF experience strongly preferred
Commitment to high productivity and patient-centered care

Education

Doctor of Physical Therapy (DPT) or equivalent

Knowledge & Skills

Expertise in SNF rehabilitation practices
Strong evaluation and treatment planning skills
Ability to thrive in a team-oriented environment
