Roles & Responsibilities
- Perform thorough assessments of patients' physical abilities
- Develop and implement individualized treatment plans
- Deliver evidence-based physical therapy interventions
- Collaborate with physicians, nursing staff, and therapists
- Monitor patient progress and adjust therapy plans as needed
Qualifications
- Graduate of an accredited Physical Therapy program
- Current state license to practice as a Physical Therapist
- CPR certification
- Experience in a SNF or geriatric rehab setting preferred
