Roles & Responsibilities

Provide compassionate physical therapy services directly in patients' homes across Yolo County
Assess and evaluate each patient's therapeutic needs, mobility, and rehabilitation potential
Develop and implement personalized treatment plans in collaboration with physicians and families
Educate patients and caregivers on therapeutic exercises, equipment use, and safety techniques
Monitor progress and communicate changes in condition to the care team promptly
Coordinate with home health aides and other professionals to support comprehensive rehabilitation
Document clinical findings and progress summaries to ensure continuity of care

Qualifications

Minimum 2 years experience as a Physical Therapist in acute, home care, or rehab settings
Current BLS certification required
Strong communication and organizational skills
Ability to work independently while thriving as part of a collaborative care team
Valid driver's license and reliable transportation for home visits

Knowledge & Skills

Physical therapy assessment and treatment planning
Home safety evaluation and adaptive equipment recommendations
Patient and family education techniques
Documentation and clinical reporting
Medicare and Medi-Cal compliance knowledge
Interdisciplinary care coordination
